The Foundation would like to recognize your organization’s donation on our website and in other materials that we distribute publicly. These materials include publications and press releases. Unless your organization wishes to have its donation remain anonymous, we ask that your organization use this form to consent to the use of your organization’s name and logo for these recognition purposes. To ensure that your organization’s name and logo are being used according with your policies, we ask that you provide us with a summary of your organization’s policies as they relate to our use of your name and logo.
In addition, the Foundation works with organizational donors through a Partner Liaison program. This program affords our donors with a forum for both receiving information on the progress of the Foundation and providing input to the Foundation to assist in improving and expanding our DEI efforts. We ask that you identify one or more individuals from your organization that have agreed to serve as your Partner Liaisons.
